Physician Offices and Home Care

An office or clinic based physician knows that some patients have a difficult time visiting their doctors regularly. In some cases, the patient may have recently fallen or had a surgery which makes transport very difficult. That's where physician offices and home care providers like us can work together. 

We work with physician offices and clinics to visit these patients in their home. We will send all visit documentation and prescription information to their offices. If the patient’s health improves to where they can make it to the office, we will transfer the patient back to their regular physician. Home Physicians also refers patients to office based practices who may not have had a primary physician and were dependent on emergency rooms or hospitals.
