Home Care and Hospice

Some patients and families who have elected hospice either do not have a regular physician or do not have one who will follow them through home care and hospice. Because we are accustomed to handling patients with complex conditions and visit them at their home, Home Physicians is a good choice. We serve as the primary care physician and work constructively with the hospice staff and resources to provide the best care possible.

We will also follow current Home Physician patients into hospice with the approval of both the patient and the hospice chosen. This coordination of home care and hospice is comforting for our patients and allows for a better transfer of information to the hospice staff.
