Direct Patient Care Health Services
in the Home

Our services include:

• Primary Care; Congestive Heart Failure, Diabetes, COPD, Asthma and Hypertension, Dementia, Alzheimer's, Arthritis, and Complex Medical Conditions
• Podiatry Care
• Wound Care
• Care Coordination for Comprehensive Laboratory, Mobile X-Rays, and Echocardiograms
• Medication management

All services are provided in the home by highly trained physicians or providers and licensed, insured and pre-screened ancillary care vendors in the comfort and safety of a patient’s home. Our goal is to have a Provider available to all qualified patients on a timely basis. Services and tests are delivered promptly per physicians orders. All billing is done directly with Medicare Part B, Medicaid, or most insurance companies. The patient may be responsible for co-pays or deductibles depending on their coverage.
