Comprehensive Health Assessments
Alegis Care, a division of Home Physicians, provides comprehensive health assessments for health plans and their members in their place of residence. Our in-home comprehensive assessments give health plan members access to quality medical care by our highly- trained providers. We accomplish this by working with the health plan and utilizing health plan-specific assessment evaluations to accurately diagnose and document conditions.
Comprehensive evaluation services are also provided in the patients’ place of residence resulting in higher patient participation, compliance, comfort and satisfaction levels. Care plans may be established at the health plan's request after the completion of the assessment. We also connect members to their Primary Care Physicians and send assessment documents to both the plan and provider.
Alegis Care ensures coding accuracy for all assessments which enables the health plan to provide optimal treatment and receive the appropriate premium. We also have the ability to provide additional medical services as well as data that is necessary for HEDIS and Star reporting.
