Terence Agustin, MD  Dr. Agustin received his medical degree at the University of Santo Tomas.  He completed his residency in Internal Medicine at Cook County Hospital in Chicago, Illinois. He is Board Qualified in Internal Medicine.

Ijaz Ahmad, MD  Dr. Ahmad is a board certified internist who completed his residency at Cook County Hospital.

Kevin Barrett, NP  NP Barrett received his Master of Science in Nursing as an Adult Nurse Practitioner from North Park University in Chicago, Illinois.  He is Board Certified as an Adult Nurse Practitioner.

Diane M. Brooks, PA  PA Brooks attended Chicago State University and completed the Physician Assistant Program at Stroger Hospital Cook County-Malcolm X College. She received her Master of Medical Science at Saint Francis University in Pennsylvania. Prior to coming to Home Physicians she had worked at Access Community Health Network, and within a private medical practice in Maywood, Illinois performing House Calls to home bound Geriatric patients, and also within the Emergency Room Department of Mount Sinai Hospital in Chicago.

Patricia Burke, MD  Dr. Burke received her medical degree from Yale University School of Medicine in New Haven, CT.  She completed her residency at the University of Virginia Hospitals in Charlottesville, Va. and most recently has worked in Primary Care with the Integrated Health Advocacy Program in Chicago and with Advocate Health Systems. She has also logged many volunteer hours serving the medically indigent population at the DuPage Community Clinic.

Kevin Carney, MD  Dr. Carney is a board certified internist who completed his residency at the University of Illinois.

Christopher CO, MD  Dr. Co received his medical degree at the University of the Philippines.  He completed his residency in Internal Medicine/Preventative Medicine at the State University of New York at Buffalo.  He is Board Qualified in Internal Medicine.

Karim Dajani, MD  Dr. Dajani is a board certified internist who completed his residency and geriatric fellowship at the University of Cincinnati.

Tracey Davenport, MD  Dr Davenport received her medical degree from Southern Illinois University School of Medicine and completed her training at Baystate Medical Center in Springfield, Massachusetts. Dr. Tracey Davenport joined Home Physicians in September of 2005 where her emphasis has been on treating the whole patient, combining medicine with health and nutritional care. Prior to coming to Home Physicians Dr. Davenport had a Private Practice that concentrated on Nursing Home Care. Dr. Davenport is Board Eligible in Internal Medicine, and is a member of the Coles County American Medical Association where she has held the offices of President, Vice President and Secretary. She is also a member of the Ethics Committee at Sara Bush Lincoln Hospital and the Theta Omega Chapter of the Alpha Kappa Alpha Sorority.

Lester Davis, MD  Dr. Davis is a board certified internist who completed his residency at Michael Reese Hospital.

Michael Fessenden, MD, MBA (Chief Medical Officer)  Dr. Fessenden is a board certified family practitioner.  Dr. Fessenden completed medical school at the University of Missouri Kansas City with a combined BA/Medical Degree.  He completed his residency at North Dakota Family Practice in Minot, North Dakota.

Augusto Guevara, MD  Dr. Guevara is a board certified internist who completed his residency and geriatric fellowship at The University of Illinois.

Giovanni Infusino, MD  Dr. Infusino received his medical degree from the University of Wisconsin at Madison.  He completed his residency in Internal Medicine at the University of Illinois at Chicago.  Dr. Infusino is Board Certified in Internal Medicine.

Georgios Karanastasis, MD  Dr. Karanastasis received his medical degree at the University of Athens Medical School in Greece.  He completed his residency in Internal Medicine at John H. Stroger, Jr. Hospital of Cook County.  He is Board Certified in Internal Medicine.

Amy Karides, NP  NP Karides received her Bachelors of Science in Nursing from Illinois Wesleyan University in Bloomington and her Master of Science in Nursing/Nurse Practitioner certification from University of Illinois @ Chicago.  She has Post Masters studies  from UIC as a Geriatric Nurse Practitioner.  Prior to joining Home Physicians NP Karides worked for Schering-Plough and managed outpatient clinic patients for a gastroenterology practice in Peoria.  She is board certified by the American Nurses Credentialing Center as both an Acute Care Nurse Practitioner and a Geriatric Nurse Practitioner.  NP Karides is a member of the Gerontological APN Association.

Shadi Latta, MD  Dr. Latta received his medical degree at the University of Aleppo.  He completed his residency in Internal Medicine at St. Joseph Hospital in Chicago, Illinois. He is Board Certified in Internal Medicine.

Mary Lohrmann, PA-C  PA Lohrmann received her physician assistant degree at The George Washington University in Washington, DC.  She is Board Certified as a physician assistant by the National Commission on certification of Physician Assistants.

Patricia Mack, ANP  ANP Mack received both her BSN and her MSN from the University of Illinois at Chicago. Before accepting this position with Home Physicians, Mack APN held the position of Staff Nurse at the Fresenius Medical Care Center in Chicago, and prior to that she held nursing positions at West Suburban Medical Center and at Direct Home Healthcare.

Premesh Malapati, MD  Dr. Malapati is a board certified internist who completed his residency Cook County Hospital.

Sharon Mangum, MD  Dr. Mangum received her medical degree at the University of Illinois.  She completed her residency in Internal Medicine at Cook County Hospital.  She is Board Certified in Internal Medicine with added qualifications in geriatrics.

Patricia McCann, NP  NP McCann received her Bachelors of Science in Nursing from St. Xavier University and her Master of Science in Nursing/Nurse Practitioner certification from University of Illinois @ Chicago.  Prior to joining Home Physician NP McCann worked for 14 years as a nurse with Little Company of St. Mary Hospital in Evergreen Park.  She is board certified by the American Nurses Credentialing Center as an Adult Nurse Practitioner and is board qualified as a Gerontological Nurse Practitioner.  She is a member of the American Academy of Nurse Practitioners.

Rachel Muthui, ANP  ANP Muthui received her BSN in Nursing from Rush University School of Nursing and her MSN from North Park University. Prior to accepting this position with Home Physicians, she rendered care at Adventist Glen Oaks Hospital in Glendale Heights along with providing care at Mount Sinai Hospital in Chicago, Rush North Shore Hospital in Skokie and Alden Valley Ridge Rehabilitation in Bloomingdale, Illinois.

David Nelson, Sr., NP  NP Nelson received his Master of Science in Nursing as a Nurse Practitioner from Purdue University -Calumet in Hammond, Indiana.  He is Board Certified as a Family Practice Nurse Practitioner.

Catherine O'Grady, NP  NP O'Grady received her BSN degree from Governor's State University and her Adult Nurse Practitioner degree from UIC - College of Nursing.  She is board certified as an Adult Nurse Practitioner through the ANCC.  NP O'Grady  holds certification from the American Board of Neuroscience Nursing and is currently employed at the University of Illinois  - Neuro ICU.

Chibiko Onyeukwu, MD  Dr. Onyeukwu received his Doctorate in Medicine from Friendship University in Moscow, Russia.  He completed his residency at St. Clare's Hospital and Health Center in New York City.  Dr. Onyeukwu has been practicing in the Bronx, New York for the past 20 years.  He is a member of the American Medical Association and New York Academy of Science.

Thomas Pawlowski, MD  Dr. Pawlowski is a board eligible family practitioner who completed his residency at Ravenswood Hospital.

Olusoji Peter, MD  Dr. Peter is board certified internist who completed his residency at Mt. Sinai Medical Queens Hospital Center.

Sam Puccinelli, PA-C  PA Puccinelli received his physician assistant degree at Midwestern University.  He is Board Certified as a physician assistant by the National Commission on Certification of Physician Assistants.

Priscilla Rivera, NP  NP Rivera received her Bachelors of Science in Nursing from West suburban College in Oak Park, IL her Master of Science in Nursing/Nurse Practitioner certification from North Park University in Chicago.  Prior to joining Home Physician NP Rivera worked as a clinical instructor at Ambria College of Nursing and as a staff nurse in the critical care unit at West Suburban Hospital.  She is board certified by the American Academy of Nurses Practitioners as an Adult Nurse Practitioner.

Faisal Saeed, MD  Dr. Saeed is a board certified internist who completed his residency at Mt. Sinai Hospital.

Elizabeth Skora, ANP  ANP Skora completed her BSN at Boston College in Chestnuthill, MA, and received her MSN from Rush University Graduate College of Nursing in Chicago. Before accepting this position at Home Physicians Skora ANP was a member of the Clinical Faculty Staff with the Department of Adult and Gerontological at Rush University College of Nursing. And prior to that she held positions at NP Care of Illinois and Crestwood Care Center as a Nurse Practitioner.

Vernedia Slaughter, FNP FNP Slaughter received her BSN in Nursing from Chicago State University and her MSN from the University of Saint Francis in Joliet, Illinois. Prior to accepting this position with Home Physicians, she was a Disease Management Nurse with McKesson Health Solutions in Park Forest and previously a Public Health Home Care Nurse with the Park Forest Health Department.

Eva Spyratos, NP  NP Spyratos received her Master of Science in Nursing as a Nurse Practitioner from DePaul University in Chicago, Illinois.  She is Board Certified as an Adult Nurse Practitioner through the American Academy of Nurse Practitioners.

Kiran Srirama, MD  Dr. Srirama is a board eligible geriatrician who completed his geriatric fellowship at the University of Illinois.

Nicholas Stiso, MD  Dr. Stiso is a board eligible internist who completed his residency at Mercy Hospital.

Greg Tomczyk, PA-C  PA Tomczyk practiced at the Hematology/Oncology clinic at the University of Chicago.   Prior to joining them he was a PA for 7 years in an internal medicine clinic in St. Charles.  He received his PA training at Midwestern University.

Alexandra Sulik, NP  NP Sulik received her Master of Science in Nursing as a Nurse Practitioner from Loyola University in Chicago, Illinois.  She is Board Certified as an Adult Nurse Practitioner.

Brian Weinstein, ANP  ANP Weinstein received his Certificate of Professional Nursing from Case Western Reserve University in Cleveland, Ohio and his MSN from North Park University. Prior to accepting this position with Home Physicians, he was the Chief Nurse Practitioner with Home Bound Health Care in Flossmoor, IL and ran NP At Home based out of Chicago.  He is Board Certified as an Adult Nurse Practitioner by the American Academy of Nurse Practitioners.

Karen M. Williams, FNP FNP Williams completed her Undergrad work at IIT, and received her BSN at Rush University College of Nursing. She received a Masters with the University of Phoenix in Nursing and received her MSN-FNP degree at DePaul University. Before accepting this position at Home Physicians she had worked previously as a General Surgery Nurse at Rush Presbyterian Hospital, as a Nurse Manager at Jackson Park Hospital and a HIV/AIDS Clinical Coordinator at Roseland Christian Health Center.

Emre Yedidag, MD  Dr. Yedidag received his medical degree at Ankara University School of Medicine in Turkey.  He completed his residency in Surgery from Indiana University School of Medicine.  He completed transplant residencies at Northwestern University and the University of Iowa.  He is Board Certified in General Surgery.

Albert Young, PA PA Young received his Bachelors of Science at Roosevelt University in Chicago and completed the Physician Assistant Program at Stroger Hospital Cook County-Wide College, Physician Assistant Program, He is also Board Certified in Primary Care by NCCPA. Prior to coming to Home Physicians he was employed by Home Health Medical in Oak Lawn, Illinois where his primary responsibility was the acute care disease management to the homebound. He was also a part of the Advocate Medical Groups clinical assessment "Pilot Episode Prevention" program.

Chunliu Zhu, MD  Dr. Zhu is a board certified family practitioner who completed his residency at Morehouse School of Medicine in Atlanta.
